Team Entry Fee = $750 per 4-person team. The number of teams is limited; reserve your place early by sending in your form and payment even if you don’t know all your players’ names. If shirt size is not received by October 18, 2017, you will receive an X-large. In the event of rainout, players will receive certificates for rounds of golf at a return date of their choosing (available times will be listed on certificate).
